Notice: Request for 1095-C Form

In accordance with the Paperwork Reduction Act, Texas Center for Arts + Academics provides employees the opportunity to request a copy of their 1095-C form for tax reporting purposes. This form provides important information about the health insurance coverage offered to you by your employer.

How to Request Your 1095-C Form:

To request a copy of your 1095-C form, please ensure you have the following information ready:
  • Your Full Name
  • Your Date of Birth
  • The Institution(s) where you are or were employed
Once you have the required information, click the link below to generate an email to our HR department. We will process your request and send the requested form to you as soon as possible.

Statutory Information:

  • The 1095-C form is issued to employees under the Affordable Care Act (ACA), which requires employers to report on the health coverage provided to employees.
  • You are eligible to request the 1095-C form if you were employed at any point during the applicable tax year.
  • As required by law, the form will be provided to you by the end of January for the previous calendar year, or within 30 days of the request, whichever is later. .
